- A clear photo of your insurance card (front and back)
- Your date of birth
- Your home address
If you are covered under a spouse or parent’s insurance, please also include:
- Their full name
- Their date of birth
- Their home address
Once we have this information, we can help you better understand your Out-Of-Network benefits and potential reimbursement.

Final reimbursement decisions are made by your insurance company.Option 2: Check on your ownYou can contact your insurance company directly by calling the member services number on the back of your insurance card or by logging into your online insurance portal.
When speaking with your insurance provider, you may want to ask:
- What is my out-of-network deductible for outpatient mental health?
- Is this deductible separate from my in-network deductible?
- How much of my out-of-network deductible have I met this year?
- What is my out-of-network coinsurance for outpatient mental health, or at what rate will I be reimbursed for out-of-network services?
- Do I need a referral from an in-network provider to see an out-of-network therapist?
You can also ask how your plan reimburses for the following therapy billing codes:
- 90837 – Individual therapy (approximately 60 minutes)
- 90834 – Individual therapy (approximately 45 minutes)
- 90847 – Couples or family therapy
